Sick Call Procedures
- The sick call procedures need to be followed by all soldiers not reporting to work or leaving work because of injury or illness.
Inform your supervisor that you are injured or ill and are going to sick call:
Obtain a DD Form 689 Individual Sick Slip from your 1st SGT. (If you can’t locate your
1st SGT, come to the Aid Station, we’ll give you a 689)
After being seen at sick call return completed form 689 to your 1st SGT.
Note: It is important that we follow this procedure to maintain proper accountability.
